EXAMPLE
Patient ADiagnosis: An 18yr old diabetic patient who has poor compliance in controlling his diabetes. The patient also doesn't like hospitals and clinics and has missed multiple clinic appointments.

Solution: Patient given four kits on his last appointment. The clinician ordered quarterly HBA1C tests for the next year. The patient received an SMS message on the day before and the morning the test needed to be taken. The patient used the painless blood withdraw device. Once complete, the patient notified the courier that the sample was ready for collection which was delivered to their local laboratory. The results went straight back to the clinician to allow him to monitor the patients' bloods remotely. The patient really appreciated the reduction of hospital visits and the reduced impact his medical condition had on his day-to-day life.
Benefits to the Patient:
- The patient can collect their blood sample in the comfort of their own home.
- No need for hospital or public transportation.
- Decreased financial burden.
- Increased likelihood of taking the sample leading to better health outcomes.
EXAMPLE
Patient BDiagnosis: Ovarian cancer with bone metastases. The patient is immobile and in significant pain. She is required to attend the oncology outpatient clinic for blood tests to determine eligibility for her next cycle of chemotherapy. She is also dependent on hospital-provided transportation.
Solution: The patient was provided with a Rempath home kit during her last chemotherapy session. The clinician ordered all necessary blood markers for the next cycle through the system. The patient receives a text reminder the day before and on the day the blood test should be completed. She uses our painless, innovative blood collection device and drops the sample into her local mailbox. The results are uploaded directly to the Rempath portal and reviewed by her local oncology team.

Benefits to the Patient:
- The patient can collect her blood sample in the comfort of her own home.
- No need for hospital or public transportation.
- Reduced strain on the patient's health from travel and waiting (especially important given her limited mobility due to bone metastases).
- Lower risk of infection (particularly if the patient is neutropenic).
- Decreased financial burden.
Benefits to Clinicians/Primary Care Practices/Hospitals:
- Increased internal laboratory capacity.
- Reduced workload and need for support staff.
- Faster turnaround times.
- Lower carbon footprint.
- Reduced infrastructure costs.
- Positive financial impact.
EXAMPLE
Patient C
Diagnosis: Juvenile Idiopathic Arthritis (JIA). 10 years old with learning difficulties. The patient is uncomfortable in hospital settings but is on long-term adalimumab therapy, which requires ongoing monitoring.
Solution: The clinician ordered a complete blood count (CBC), liver enzymes, and serum creatinine through the Rempath portal. Two Rempath kits were provided during the last appointment to test these markers three months apart. The patient preferred to use our blood collection device due to its simplicity and pain-free experience.
Benefits to the Patient: Both the patient and his parents are much more at ease, as the patient no longer has to make frequent hospital visits and feels more comfortable overall.
Benefits to the Hospital:
- Lower outpatient volumes.
- Financial savings.
- Continued compliance with monitoring protocols.
